Services for Adults with Developmental Disabilities
Transitions is dedicated to providing person-centered support that recognizes the inherent dignity and worth of every person, regardless of ability. We ensure that everyone receiving our services is actively involved in all decision-making and planning concerning their lives.
In Adult Services, we collaborate with the person, their families, and their identified support networks, to develop strategies designed to help them identify their own personal goals.
Our Adult Services program serves adults who:
- Are at least 18 years of age
- Have a diagnosed developmental disability
- Receive funding from the Government of Alberta’s Persons with Developmental Disabilities (PDD) program.
- Reside in St. Albert
Adult services – What we offer
This includes either:
- 24/7 in-home support for those residing in their own homes; or
- Those living with their family independently, who only require 5 to 40 hours of weekly support to access their community and participate in planned activities.
We strive to meet the unique needs and desires of each person we serve. Whether they are living in the family home or community, we can assist with seeking gainful employment or volunteer opportunities; fostering meaningful relationships; navigating public transportation; developing home and life skills; budgeting and supporting them to discover their own treasured places in their community
Our team utilizes community resources to connect people to various professionals and programs, including but not limited to, medical professionals, therapists, grief and loss support groups, post-secondary education and Transitions (or other) learning sessions.
Transitions can also support individuals and their guardians with exploring and identifying housing that fits their budget and support needs. In the current housing climate, obtaining affordable housing often requires our team to help identify a roommate with similar interests and lifestyle.
Intake Process
The Adult Services Program Supervisors collaborate with the Manager of Adult Services, the Director of Children and Adult Services, the Coordinator of Quality Assurance and Program Support to review a registry provided by PDD of people seeking support. Together they explore any potential fit for our services based on specific criteria and current program capacity.
When a person is identified as a potential fit for a current or new vacancy within Transitions, our team contacts the PDD caseworker to express our interest and invite the person and/or their guardian(s) to an initial meeting.
The purpose of the initial meeting is twofold: (1) it provides an opportunity for the person and/or their guardian(s) to learn about Transitions’ services and values; and (2) it allows the Transitions team to assess if there is alignment between the person’s support needs and what Transitions can offer. This happens by ensuring all parties are transparent and provide full disclosure of any potential challenges.
During this meeting, no decisions are made. The person and/or guardian can take the appropriate time to consider whether Transitions is the right fit for them; and can reach out to the Supervisor or Manager with any additional questions they may have. This process may also include meeting potential roommates (if applicable) to determine compatibility.
If both Transitions and the person and/or guardian elect to proceed with securing services, Transitions will formally request approval from PDD. Once approved by PDD, services can commence on the agreed start date.
