Since our founding, Transitions has been dedicated to building new tomorrows for individuals with disabilities from birth to end-of-life. As a non-profit organization, our purpose is rooted in a deep commitment to offering person- and family-centered services that empower individuals to lead fulfilling lives.

From the beginning, our approach has been clear: we believe that creating an atmosphere of fun and creativity leads to better outcomes for everyone. This belief drives our teams to strive for excellence every day, fostering an environment where both the people we serve and our employees can thrive.

Located in St. Albert, Alberta, Transitions proudly serves our local community, extending our reach to various areas of Edmonton and Sturgeon County. Within our purpose is the goal to provide services that enable individuals to enjoy the independence and quality of life they choose. We focus on building upon each person’s strengths and abilities, ensuring that they are supported to be leaders of their own lives, form meaningful relationships, and engage fully as citizens with all the rights and responsibilities that come with it.
